Diabetes
GOAL
The goal of the ADI program is to address the high rates of diabetes and its complications. An important part of the goal is to provide members of the community with opportunities to design, develop, and participate in projects to address their diabetes within their community.
OBJECTIVES
The objectives of the ADI program of the TA Health involve:
- Providing diabetes care and treatment services to First Nations and/or Inuit living in Tootinaowaziibeeng Treaty Reserve; and/or
- Support diabetes prevention and health promotion activities to increase awareness of diabetes and its complications, create awareness of the benefits of healthy living, encourage the integration of traditional methods and practices with western-based approaches, and focus on the need to educate youth to decrease the likelihood of the next generation suffering from diabetes; and/or
- Provide lifestyle support services to people living with diabetes and their families/care providers, in coping with the consequences of having a chronic, potentially debilitating, or life threatening disease.
- Implement the activities (prevention and promotion, care and treatment, lifestyle supports) identified within the community diabetes work plan;
- Advocate, promote and support the utilization of the Manitoba Diabetes Care Recommendations by all health care providers;
- Will utilize the multi-sectoral team approach throughout the delivery of diabetes prevention and promotion, care and treatment, and lifestyle supports activities to ensure a holistic approach
- To collaborate with and encourage community-based linkages with other programs, communities, tribal councils, provincial organizations and non-governmental organizations throughout the implementation of the diabetes work plan;
- Collaborate with and utilize the community diabetes action committee for their ongoing guidance and support with the community diabetes plan; and
- Provide and/or support capacity building opportunities for community human resources working within the community.
