Home & Community Care
Goal:
The main goal of the Home and Community Care Program is to assist on-reserve clients to live on the reserve as independently as possible, preserving, and encouraging enhancement of the support provided by the family, relatives, other people, and other programs, through provision of essential services and supportive care service in a professional manner and use of ethical guidelines in delivery of services.
Objective:
- Providing a single access point for health care support services for individuals needing home health care services.
- Providing client with a consistent, comprehensive, and client-centred assessment of need through integration of health promotion goals and practices in care plans and in provision of health care.
- Teaching client, families and others to provide care to promote self-reliance and efficient use of resources.
- Coordinating the medical supplies and equipment needs of clients.
- Professional and certified staff personnel utilized in delivery of services.
- Develop the capacity to evaluate program effectiveness by developing an evaluation framework and plan.
- Develop and implement methods to determine the relative priority of cline tends based on assisting persons who are highest risk.
Focus:
The Tootinaowaziibeeng Anishinabe Home & Community Care Program is an assessed service available for certain or specified Home Care Support to individuals who can continue to live independently in a safe and practical manner within their own home environment. The assessment process includes the individual; informal care give, family and Home Care Staff.
